Elements of Competency Performance Criteria              
Element: Identify patient information needs.
  • Review patient documentation to identify the medication the patient is taking.
  • Clarify patient information needs with a pharmacist or an authorised person.
  • Identify and plan to meet patient special needs which may impact on the effectiveness of communication.
  • Use standard assessment tools to identify a patient’s ability to manage their medications.
  • Discuss information needs with the patients and carers and refer needs outside of scope of role to a pharmacist or an authorised person.
       
Element: Gather and prepare information to meet patient needs.
  • Access reference material to gather the required information.
  • Ensure information is in a suitable format for the patient to understand.
  • Select appropriate modes of communication to suit the purpose and context of the information being provided.
       
Element: Provide specific or prepared information to patients.
  • Provide prepared information to patients when requested by a pharmacist or an authorised person.
  • Present relevant information to the patient clearly and in sufficient detail to meet the needs of the patient.
  • Reinforce the key messages provided to patients by the authorised person or through Consumer Medicines Information (CMI) and refer needs outside of scope of role to a pharmacist or an authorised person.
  • Exercise appropriate discretion and confidentiality and seek advice and assistance from a pharmacist or an authorised person for unresolved concerns or issues.
  • Negotiate appointments with patient and staff for the provision of information to the patient in accordance with organisational policies and procedures.

  • Detect new patient admission and notify a pharmacist or an authorised person.
  • Record patient’s medication brought into hospital or to the health services provider.
  • Communicate medication supply information to internal medical, nursing staff and community providers.
  • Obtain discharge planning information from relevant source.
  • Prepare information for health professional for verification by a pharmacist or an authorised person.
  • Transfer information to health professional after verification by a pharmacist or an authorised person.

  • Confirm with a pharmacist or an authorised person that patient information needs have been met.
  • Record details of information provided to the patient in accordance with organisational procedures.

Assessment Tasks

Copy and paste from the following data to produce each assessment task. Write these in plain English and spell out how, when and where the task is to be carried out, under what conditions, and what resources are needed. Include guidelines about how well the candidate has to perform a task for it to be judged satisfactory.
Copy and paste from the following performance criteria to create an observation checklist for each task. When you have finished writing your assessment tool every one of these must have been addressed, preferably several times in a variety of contexts.
